A study on the imaging features and prognostic factors of acute non-large vessel occlusion stroke evaluated using 3D Slicer

基于3D Slicer评估急性非大血管闭塞型脑梗死的影像学特点及其预后因素研究

Abstract: Objective To investigate the imaging characteristics of acute non-large vessel occlusion ischemic stroke using 3D Slicer software and its prognostic influencing factors. Methods A retrospective analysis was conducted on the clinical data of 300 patients with acute non-large vessel occlusion ischemic stroke treated in our hospital from October 2022 to February 2024.The software 3D Slicer was utilized to calculate the infarct volume,maximum infarct cross-sectional area,and maximum infarct long diameter of patients with different infarct locations,followed by statistical analysis.Univariate and multivariate analyses were performed to identify factors related to the prognosis of patients with acute non-large vessel occlusion ischemic stroke. Results Among 300 patients with acute non-large vessel occlusion cerebral infarction,the infarction sites were compared with each other.Sporadic cerebral infarction was the dominant type,accounting for approximately 40.7%,followed by basal ganglia infarction at approximately 27.3%,and parieto-occipital lobe infarction was the least,accounting for approximately 5%.When comparing the infarction volumes of different infarction sites based on the calculation by 3D Slicer software,there was no statistically significant difference(P>0.05).However,when comparing the maximum infarction cross-sectional area and the maximum infarction long diameter,the difference was statistically significant(P<0.05).Regression analysis revealed that the infarction volume was an independent risk factor affecting the 90-day prognosis of the patients. Conclusion The 3D Slicer software can accurately calculate the volume of acute non-large vessel occlusion ischemic stroke,and infarct volume is an independent risk factor influencing the 90-day prognosis of patients with acute non-large vessel occlusion ischemic stroke.

摘要: 目的 探讨3D Slicer软件在急性非大血管闭塞型脑梗死中的影像学特点及其预后影响因素。方法 回顾性分析2022年10月至2024年2月在我院治疗的急性非大血闭塞型脑梗死300例患者的临床资料。基于3D Slicer软件计算不同梗死部位患者的梗死体积、最大梗死截面面积及最大梗死截面长径并统计分析,采用单因素、多因素分析影响急性非大血管闭塞性脑梗死患者预后的相关因素。结果 300例急性非大血管闭塞型脑梗死患者梗死部位相互比较,其中以散发性脑梗死为主约占40.7%,其次为基底节区脑梗死,约占27.3%,顶枕叶梗死最少约占5%;基于3D Slicer软件计算不同梗死部位的梗死体积之间比较,差异均无统计学意义(P>0.05);而最大梗死截面面积以及最大梗死长径相互比较,差异有统计学意义(P<0.05)。回归分析显示,梗死体积是影响患者90天预后的独立危险因素;结论 3D-Slicer软件可以精准的计算急性非大血管闭塞型脑梗死体积,梗死体积是影响急性非大血管闭塞型脑梗死患者90天预后的独立危险因素。
