单孔双管引流在慢性硬膜下血肿钻孔引流术中的应用

Abstract: Objective Analyze and study the application effect of single port and double tube drainage in drilling and drainage of chronic subdural hematoma.Methods Select 96 patients with chronic subdural hematoma treated in our hospital from October 2015 to October 2020.According to the odd and even numbers of the last digit of the hospitalization number,50 patients in the odd group used single port and double tube drainage during the drilling operation,46 patients in the even group used traditional single port and single tube drainage during drilling,the same treatment scheme was given after the operation.Finally,the operative effects,operative time,treatment costs and complications of the two groups were statistically analyzed.Results There was no statistically significant difference in the operative efficiency,the incidence of brain injury,the incidence of incisional cerebrospinal fluid leakage,the incidence of epilepsy,the incidence of incision infection,the indwelling drainage time,and the treatment cost between the two groups.In the comparison of postoperative intracranial gas volume and the incidence of hematoma recurrence,the single port double tube drainage group was better than the single port single tube drainage group,and the difference was statistically significant.In the comparison of operation time,the single port single tube drainage group was better than the single port double tube drainage group,and the difference was statistically significant.Conclusion In the drilling operation treatment of chronic subdural hematoma,both drainage schemes are effective.in which single port double tube drainage can better reduce the postoperative intracranial gas volume and the incidence of hematoma recurrence.

摘要: 目的 分析研究单孔双管引流在慢性硬膜下血肿钻孔引流术中的应用效果。方法 选择2015年10月至2020年10月5年之间我院收治的96例慢性硬膜下血肿患者,按照患者住院号末位数字奇、偶数分组,奇数组的50例患者钻孔术中使用单孔双管引流,偶数组的46例患者钻孔术中使用传统的单孔单管引流,术后均给予相同的治疗方案进行治疗,最后将两组患者的手术效果、手术时间、治疗费用及并发症情况进行统计分析。结果 两组患者的手术有效率、脑损伤发生率、切口脑脊液漏发生率、癫痫发生率、切口感染发生率、留置引流时间、治疗费用的比较差异无统计学意义。术后颅内积气量、血肿复发发生率的对比中,单孔双管引流组要优于单孔单管引流组,差异有统计学意义。在手术时间的对比中,单孔单管引流组要优于单孔双管引流组,差异有统计学意义。结论 在慢性硬膜下血肿的钻孔手术治疗中,两种引流方案均为有效方案,其中单孔双管引流更能降低术后颅内积气量及血肿复发发生率。
