Abstract: Objective Dexmedetomidine (DEX) is a highly selective drug α 2-adrenergic receptor agonists have significant analgesic and sympathetic blocking effects.This study intends to use intraoperative MRI scanning in our hospital to locate and judge the brain areas involved in DEX promoting analgesia under general anesthesia. Methods 12 patients with right epilepsy undergoing radiofrequency ablation under intraoperative magnetic resonance guidance from January to March 2022 were selected as the research objects.The included subjects were randomly divided into DEX group(n= 6) and control group (control group) (n= 6).DEX group:1.5ug/kg/h DEX was continuously pumped before anesthesia induction.After continuous infusion for 15min,anesthesia induction was carried out,and then scanning was started after continuous infusion for 30min.Control group:continuous pumping of normal saline with the same liquid velocity before anesthesia induction,continuous infusion for 15 minutes,anesthesia induction,and scanning after continuous infusion for 30 minutes.The changes and differences of intraoperative magnetic resonance scanning Results between the two groups. Results Total of 12 cases were included.There was no significant difference in the changes of preoperative general conditions and intraoperative vital signs of all patients.The activity of amygdala nucleus in the right hemisphere in DEX group was significantly lower than that in the control group. Conclusion The target ofDEX's analgesic effect is in the amygdala.

摘要: 目的 右美托咪定(简称右美)是一种高度选择性的α2-肾上腺素能受体激动剂,具有显著的镇痛和交感神经阻滞作用。本研究拟采用利用我院术中核磁共振扫描来定位判断在患者全麻条件下右美镇痛作用所涉及的脑区。方法 选取2022年1月至3月拟行术中磁共振引导下右侧癫痫射频消融术患者12例患者为研究对象。将纳入的研究对象随机分为右美组( DEX组) (n=6)和对照组(Control组)(n=6)。 DEX组:麻醉诱导前持续泵入1.5 ug/kg/h右美,持续输注15 min后麻醉诱导,后持续输注30 min后开始扫描。 Control组:麻醉诱导前持续泵入同等液速生理盐水,持续输注15 min后麻醉诱导,后持续输注30 min后开始扫描。比较并分析两组术中磁共振扫描结果变化及差异,用于找到右美独特的镇痛靶点。结果 共纳入12例,所有患者术前一般情况及术中生命体征变化差异无统计学意义。右美组在右侧半球杏仁核核团的活性明显低于对照组。结论 右美镇痛作用的靶点在杏仁核。
