The risk factors of cerebrospinal fluid infection in children with posterior fossa tumorsafter operation

儿童脑肿瘤术后颅内感染的危险因素分析

Abstract: Objective To investigate the risk factors of cerebrospinal fluid infection in children with posterior fossa tumors after operation.Methods 96 children with posterior fossa tumors in our hospital from October 2020 to October 2024 were selected in the Department of Neurosurgery,The Affiliated Children's Hospital of Xi'an Jiaotong University and divided into infection group (18 cases) and non infection group (78 cases) according to whether intracranial infection occurred after operation.The clinical data of the two groups were compared,and the risk factors of intracranial infection were analyzed by multivariate logistic regression.Results The incidence of intracranial infection was 18.75% in the 96 cases of children.Univariate analysis showed that there was no significant differece in gender,age,weight,preoperative albumin(P>0.05).Univariate analysis showed that there had significant difference in tumor volumes,operation time,external ventricular drainage,blood loss and postoperative cerebrospinal fluid leakage(P>0.05).Multivariate Logistic regression analysis showed that tumor volumes,operation time,external ventricular drainage,blood loss and postoperative cerebrospinal fluid leakage were risk factors of intracranial infection.After lumbar puncture and antibiotic treatment,intracranial infection was cured.The course of treatment was about 2 to 6 weeks.Conclusion The occurrence of postoperative intracranial infection in children with posterior fossa tumors were affected by many factors,among which tumor volumes,operation time,operation blood loss,external ventricular drainage and cerebrospinal fluid leakage were risk factors of postoperative infection.Clinicians should pay more attention to avoid intracranial infection.

摘要: 目的 探讨儿童后颅窝肿瘤术后颅内感染的危险因素。方法 选择于2020年10月至2024年10月在西安交通大学附属儿童医院神经外科接受诊治的儿童后颅窝肿瘤96例,根据术后是否发生颅内感染分为感染组 18 例与未感染组78例。对比2组患儿的临床资料,采用多因素 Logistic 回归分析颅内感染的危险因素。结果 96例儿童脑肿瘤术后中,颅内感染发生率18.75%。单因素分析显示,两组患儿性别、年龄、体重、术前白蛋白差异无统计学意义(P>0.05)。两组患儿在肿瘤体积、手术时间、脑室外引流、手术出血量和术后脑脊液漏方面差异有统计学意义(P<0.05)。多因素Logistic回归分析显示,肿瘤体积、手术时间、脑室外引流、手术出血量和术后脑脊液漏为导致术后颅内感染的危险因素,差异有统计学意义(P<0.05)。颅内感染经腰椎穿刺术、抗生素治疗后,颅内感染均治愈,疗程约2周至6周。结论 儿童后颅窝肿瘤术后颅内感染的发生受多种因素影响,其中肿瘤体积、手术时间、手术出血量、脑室外引流和脑脊液漏均为术后感染危险因素,临床医师积极重视,避免出现颅内感染。
