比较听性脑干反应V波潜伏期耳间差值、头颅MRI与新生儿高胆红素血症听力损害相关性研究

A comparative study of the correlation between the inter-aural latency difference of brainstem auditory evoked potential wave V,skull MRI and hearing impairment in neonates with hyperbilirubinemia

摘要: 目的 研究听性脑干反应V波潜伏期耳间差值、头颅MRI与新生儿高胆红素血症听力损害相关性。方法 选取2019年11月至2022年1月在中国科学技术大学附属第一医院新生儿科住院治疗的确诊为高胆红素血症患儿共计225例,并对所有患儿均完善肝功能检查、BAEP和头颅MRI检查。结果 重症组与轻症组患儿在性别、胎龄、黄疸发生日龄、出生体重、出生方式方面,两组患儿差异无统计学意义(P>0.05);重症组患儿的听阈值和V波潜伏期耳间差值的异常比例均要高于轻症组患儿(P<0.05);重症组患儿的V波ILD明显高于轻症组患儿(P<0.05);轻症组患儿中,日龄11~14 d亚组患儿的单耳V波潜伏期明显低于日龄7~10 d亚组,而在Ⅴ波ILD方面两亚组无明显差异(P>0.05)。结论 ILD和头颅MRI对高胆红素血症新生儿的听力损伤有较好的早期诊断价值。

Abstract: Objective To study the correlation between the inter-aural latency difference of brainstemauditoryevokedpotentialwaveV,skull MRI and hearing impairment in neonates with hyperbilirubinemia.Methods A total of 225 children with neonatal hyperbilirubinemia who were admitted to the Department ofNeonatologyof Anhui Provincial Hospital from November 2019 to January 2022 and underwent BAEP examination were selected,and liver function examination,BAEP examination and head MRI examination were performed.Results There were no significant differences in gender,gestational age,birth weight,birth mode and age of jaundice occurrence between the two groups (P> 0.05).The proportion of abnormal hearing threshold and abnormal ILD in the severe group was higher than that in the mild group (P<0.05),and the ⅴ wave ILD in the severe group was higher than that in the mild group (P<0.05).The incubation period of ⅴ wave in mild group was longer than that in mild group at 7-10 days of age (P<0.05),while there was no significant difference in THE ILD between the two groups (P>0.05).Conclusion ILD and SkullMRI have good value in early diagnosis of hearing impairment in neonates with hyperbilirubinemia.
