迷走神经电刺激治疗青少年/成人药物难治性癫痫的疗效观察

Clinical observation of the efficacy of vagus nerve stimulation in treating Drug-Resistant epilepsy in adolescents and adults

摘要: 目的 本研究旨在通过多中心、大样本的临床研究,评估迷走神经电刺激(VNS)治疗青少年和成人药物难治性癫痫的疗效。旨在为该疗法在中国的临床应用提供依据,并深入探讨其长期疗效。方法 本研究于2022年1月到2024年11月期间,在全国9家癫痫中心招募了206名药物难治性癫痫患者,所有患者接受了VNS治疗,并在术后3个月、6个月和12个月进行了随访,记录癫痫发作的频率。使用SPSS软件对数据进行统计分析。结果 研究结果显示,在3个月随访时,患者的癫痫发作减少率为33.34%;在6个月和12个月时分别为40.36%和51.26%。在术后3个月、6个月和12个月,改善50%以上的患者比例分别为23.79%、40.80%和53.77%。结论 本研究表明,VNS对药物难治性癫痫患者具有显著的治疗效果,尤其是在术后6个月和12个月时,癫痫发作的减少率逐渐提高。研究结果为VNS疗法在中国的临床应用提供了支持,同时也强调了长期随访和个性化治疗调整对疗效的重要性。

Abstract: Objective This study aims to evaluate the efficacy of Vagus Nerve Stimulation (VNS) in treating drug-resistant epilepsy in adolescents and adults through a multicenter,large-sample clinical trial.The goal is to provide evidence for the clinical application of this therapy in China and to explore its long-term efficacy. Methods This study recruited 206 patients with drug-resistant epilepsy from nine epilepsy centers across China between January 2022 and November 2024.All participants underwent VNS treatment and were followed up at 3,6,and 12 months post-surgery to record the frequency of epileptic seizures.Statistical analysis was performed using SPSS software. Results The results showed a 33.34% reduction in seizure frequency at the 3-month follow-up;at 6 and 12 months,the reduction rates were 40.36% and 51.26%,respectively.The proportion of patients with over a 50% improvement in seizure frequency was 23.79% at 3 months,40.80% at 6 months,and 53.77% at 12 months post-surgery. Conclusion This study demonstrates that VNS has significant therapeutic effects on drug-resistant epilepsy,particularly with increasing seizure reduction rates at 6 and 12 months post-treatment.The findings support the clinical application of VNS in China and emphasize the importance of long-term follow-up and personalized treatment adjustments for optimal efficacy.
