MRI测定Ve、Ktran、rADC及tADC值对颅脑胶质瘤的分级诊断及对患者预后不良的预测

摘要: 目的 研究MRI测定血管外细胞外间隙容积比(Ve)、容积转换常数(Ktrans)、相对表观扩散系数(apparent diffusion coefficient,ADC)值(rADC)及肿瘤ADC值(tADC)对颅脑胶质瘤的分级诊断及对患者预后不良的预测价值。方法 本研究为前瞻性研究,以本院2020年2月到2023年2月在本院诊断的颅脑胶质瘤患者120例作为研究对象,依据世界卫生组织的病理学分级标准,其中低级别脑胶质瘤患者55例,高级别脑胶质瘤患者65例,比较不同预后以及不同分级患者的Ve、Ktran、rADC及tADC差异,研究Ve、Ktran、rADC及tADC联合诊断对于不同预后以及不同分级的预测价值。结果 高级别组患者的Ve(t=24.003,P=0.000)、Ktran(t=12.633,P=0.000)高于低级别组,rADC(t=9.951,P=0.000)及tADC(t=14.050,P=0.000)值低于低级别组;预后不良组患者的Ve(t=24.003,P=0.000)、Ktran(t=12.633,P=0.000)高于预后良好组,rADC(t=9.951,P=0.000)及tADC(t=14.050,P=0.000)值低于预后良好组,复发组患者的Ve(t=5.830,P=0.000)、Ktran(t=15.548,P=0.000)高于非复发组,rADC(t=21.143,P=0.000)及tADC(t=14.893,P=0.000)值低于非复发组(P<0.05);Ve、Ktran、rADC及tADC值联合诊断对高级别组以及预后不良组诊断灵敏度显著高于单独诊断;通过ROC曲线分析,Ve、Ktran、rADC及tADC值联合诊断对高级别组以及预后不良组诊断的曲线下面积显著高于单独检测。结论 MRI测定Ve、Ktran、rADC及tADC值对颅脑胶质瘤的鉴别诊断及预后不良具有显著的临床意义,建议临床推广。

Abstract: Objective To study the value of Ve,Ktran,rADC and tADC measured by MRI in the grading diagnosis of cranial glioma and the prediction of poor prognosis.Methods This is a prospective study.120 patients with cerebral glioma diagnosed in our hospital from February 2020 to February 2023 were selected as research objects.According to the pathological grading standards of the World Health Organization,there were 55 patients with low-grade glioma and 65 patients with high-grade glioma.The difference of Ve,Ktran,rADC and tADC among patients with different prognosis and different grades was compared,and the predictive value of the combined diagnosis of Ve,Ktran,rADC and tADC for different prognosis and different grades was studied.Results Ve (t=24.003,P=0.000) and Ktran (t=12.633,P=0.000) in high grade group were higher than those in low grade group,rADC (t=9.951,P=0.000) and tADC (t=14.050,P=0.000) were lower than those in low grade group.Ve (t=24.003,P=0.000) and Ktran (t=12.633,P=0.000) in the poor prognosis group were higher than those in the good prognosis group,while rADC (t=9.951,P=0.000) and tADC (t=14.050,P=0.000) were lower than those in the good prognosis group.Ve (t=5.830,P=0.000) and Ktran (t=15.548,P=0.000) in relapsing group were higher than those in non-relapsing group,rADC (t=21.143,P=0.000) and tADC (t=14.893,P=0.000) were lower than those in non-relapsing group (P<0.05).The combined diagnosis sensitivity of Ve,Ktran,rADC and tADC values was significantly higher than that of single diagnosis in the high-grade group and the poor prognosis group.According to ROC curve analysis,the combined diagnosis of Ve,Ktran,rADC and tADC values for the diagnosis area under the curve of the high grade group and the poor prognosis group was significantly higher than that of the single detection.Conclusion MRI determination of Ve,Ktran,rADC and tADC values has significant clinical significance for differential diagnosis and poor prognosis of cerebral glioma,and it is recommended to be popularized in clinic.
