振幅整合脑电图联合MRI对足月窒息新生儿缺氧缺血性脑病的诊断价值

The diagnostic value of amplitude integrated electroencephalography combined with MRI for neonatal hypoxic-ischemic encephalopathy in full-term asphyxia

摘要: 目的 探究振幅整合脑电图(amplitude-integrated electroencephalogram,aEEG)和磁共振成像(magnetic resonance imaging,MRI)在足月窒息新生儿缺氧缺血性脑病(hypoxic-ischemic encephalopathy,HIE)诊断中的应用价值。方法 以本院2022年1月至2023年9月收治的足月窒息新生儿140例为研究对象,按相关诊断标准将患儿分为HIE组和非HIE组,分析aEEG分度和MRI综合评分对窒息患儿发生HIE的影响程度。结果 HIE的发生率为29.29%;不同分度患儿间Apgar评分差异有统计学意义(F=37.420,P<0.001),且相关性分析显示aEEG分度与Apgar评分呈中度负相关(r=-0.580,P<0.001);HIE组患儿MRI综合评分和aEEG分度异常率均高于非HIE组(均P<0.05);Logistic回归分析表明,足月窒息患儿aEEG异常及MRI综合评分升高是HIE发生的独立危险因素(P<0.05);aEEG分级、MRI综合评分以及二者联合对预测窒息新生儿HIE的AUC分别为0.737、0.721和0.826。结论 aEEG异常与MRI综合评分是窒息新生儿HIE的影响因素,两者联合对足月窒息新生儿HIE有较高的诊断价值,值得临床作进一步推广。

Abstract: Objective To investigate the application value of amplitude-integrated electroencephalography (aEEG) and magnetic resonance imaging (MRI) in the diagnosis of hypoxic-ischemic encephalopathy (HIE) in full-term asphyxiated neonates. Methods 140 full-term asphyxiated neonates admitted to our hospital from January 2022 to September 2023 were selected as the research subjects.The neonates were divided into HIE group and non-HIE group based on relevant diagnostic criteria.The influence of aEEG grading and MRI composite score on the occurrence of HIE in neonates with asphyxia was analyzed. Results The incidence of HIE was 29.29%;There was a statistically significant difference in Apgar score among children with different grades (F=37.420,P<0.001),and the correlation analysis showed that aEEG grade was moderately negatively correlated with Apgar score (r=-0.580,P<0.001);The abnormal rates of MRI comprehensive score and aEEG grading in HIE group were higher than those in non HIE group (all P<0.05);Logistic regression analysis showed that abnormal aEEG and elevated MRI comprehensive score were independent risk factors for HIE in full-term asphyxia children (P<0.05);The AUCs of aEEG grading,MRI comprehensive score and their combination for predicting HIE in asphyxiated neonates were 0.737,0.721 and 0.826,respectively. Conclusion aEEG abnormalities and MRI comprehensive score are the influencing factors of HIE in asphyxiated neonates.The combination of aEEG and MRI has high diagnostic value for HIE in full-term asphyxiated neonates,which is worthy of further clinical promotion
