脑外伤性弥漫性轴索损伤和血管功能的核磁成像空间关系分析

摘要: 目的 通过观察创伤性脑损伤(TBI)患者的扩散加权磁共振成像(DW-MRI)中扩散峰度成像(DKI)的参数与脑血流量(CBF)的关系,评估两者的空间关联。方法 纳入2020年1月至2020年12月就诊张家口市第一医院并拟行MRI的住院TBI患者(研究组)和相匹配的健康者(对照组)各30例,收集研究组患者损伤后5~19天和对照组的DKI完整性信息和动脉自旋标记测量的脑血流量。提取感兴趣区域(ROIs)的平均峰度(MK)、峰度各向异性(KA),各向异性分数(FA)及CBF的平均值。从健康对照者生成归一化z评分图进行两组的比较及其与CBF的空间定位关系。结果 30例TBI患者的全脑、脑白质容积均与CBF成正相关(r=0.500、r=0.818,均P<0.01),进一步分析,TBI患者的血流异常主要表现在海马、丘脑和杏仁核、颞回,其中其两侧颞回较对照组下降了10%左右。DKI参数提示TBI患者在大脑白质内存在广泛的DAI,多发生于大脑脚、胼胝体、左右内囊前后肢及桥脑等,两组间比较,实验组患者的指标明显低于对照组(P<0.05),在ROIs区域中,约3.83%的异常KA区域也有异常的MK值,在CBF异常的区域中,MK异常和KA异常存在分别占6.36%和7.64%,异常FA区域与CBF异常重叠9.87%。结论 DKT的参数可显示脑组织DAI微观结构的变化,DAI微结构的损伤与脑血管灌注存在有限的关联。

Abstract: Objective To evaluate the spatialcorrelationbetweendiffusion kurtosis imaging (DKI) andcerebralbloodflow (CBF) of diffusion-weighted magnetic resonance imaging (DW-MRI) in patients with traumatic brain injury (TBI).Methods 30 cases of patients with TBI (Research group) and 30 cases of matched healthy volunteers (Control group) were enrolled in the first hospital of Zhangjiakou city from January,2020 to December,2020.Axonalintegrityof DKI examination and CBF measured by arterial spin labeling (ASL) were performed 5 to 19 days enrolledin Research groups of patients after injury and Control patients.Mean values of mean kurtosis (MK),Kurtosis anisotropy (KA) and anisotropy fraction (FA),CBF were extracted for pre-determined regions of interest (ROIs).Normalized z-score maps were generated from the pool of healthy controls to compare the two groups and their spatial positioning relationship with CBF.Results whole brain,white matter volume of 30 patients of TBI were positively correlated with CBF (r=0.500,r=0.818,both P<0.01).Further analysis showed that abnormal blood flow in TBI patients was mainly presented in the hippocampus,thalamus,amygdala and temporal gyrus,among which,the CBF of bilateral temporal gyrus decreased by 10% approximately compared with the Control groups.DKI parameters suggested that extensive DAIwere observed in the white matter of the brain in TBI patients,mostly in the foot of the cerebrum,corpus callosum,anterior and posterior limbs of the left and right capsule,pons and so on.Compared with two groups,the indexes of the Research group were significantly lower than those of the Control group (P<0.05),in the ROIs area,about 3.83% of the abnormal KA area also had abnormal MK value.In the abnormal area of CBF,abnormal MK and KA accounted for 6.36% and 7.64%,respectively,and the abnormal FA region overlaps with CBF by 9.87%.Conclusion Change of DAI microstructure in brain tissue were showed from parameters of DKI,and there is limited correlation between DAI microstructure injury and cerebral vascular perfusion.
