无先兆偏头痛发作间期的TSEP研究

Trigeminal somatosensory evoked potential study during interictal period in patients with migraine without aura

摘要: 目的 探讨无先兆偏头痛(MO)患者发作间期三叉神经体感诱发电位(TSEP)的变化及其意义。方法 选取2020年10月至2022年6月在中国科学技术大学附属第一医院确诊的无先兆偏头痛患者52例,健康对照(HC)52名为该院同期健康体检正常人群。所有受试者进行TSEP检测,将各波(N13、P19和N30)的潜伏期、峰间期、峰-峰幅度及感觉阈值作为TSEP检测的分析指标,比较两组人群的TSEP各指标差异。进一步根据病程年限及疼痛程度对MO组分亚组,比较亚组间TSEP指标差异。结果 MO组TSEP各波潜伏期均显著长于HC组,差异有统计学意义(P<0.001)。在病程亚组间,随着病程的延长,TSEP的感觉阈值呈下降趋势,各波波幅呈上升趋势,其中左侧感觉阈值、左侧N30/P19波幅及右侧N13/P19波幅差异具有统计学意义(P<0.05)。在疼痛程度亚组间,两组患者TSEP指标差异无统计学意义(P>0.05)。结论 无先兆偏头痛患者发作间期存在TSEP异常,持续时间较长的偏头痛可能会影响皮质和脑干三叉神经通路的反应性和兴奋性。

Abstract: Objective To investigate the changes and significance of trigeminal somatosensory evoked potentials (TSEP) in patients with migraine without aura (MO) during interictal period.Methods 52 patients with migraine without aura diagnosed in the First Affiliated Hospital of University of Science and Technology of China from October 2020 to June 2022 were enrolled as MO group and 52 healthy cases were also included as healthy control group(HC).All subjects were examined by TSEP.The latency of each wave (N13,P19 and N30),peak interpeak period,peak-to-peak amplitude and sensory threshold were taken as the analytical indexes of TESP,and the differences of TSEP indexes between the two groups were compared.Furthermore,according to the course of disease and the degree of pain,the differences of TSEP indexes among MO subgroups were compared too.Results The latency of TSEP in MO group was significantly longer than that in HC group,and the difference was statistically significant (P<0.001).Among the subgroups of the course of disease,the sensory threshold of TSEP decreased and the amplitudes of each wave increased with the prolongation of the course of disease.The difference in left sensory threshold,left N30/P19 amplitude and right N13/P19 amplitudewas statistically significant (P<0.05).Between subgroups of pain degree,there was no significant difference in TSEP between the two groups (P>0.05).Conclusion The TSEP indexes were abnormal in patients with migraine without aura.Migraine with longer duration may affect the reactivity and excitability of trigeminal pathway in cortex and brainstem.
