儿童颅骨生长性骨折的临床特征和单中心治疗体会

摘要/Abstract

摘要： 目的分析儿童颅骨生长性骨折的发病机制、临床特点并探讨其外科手术预后。方法收集并分析西安交通大学附属儿童医院神经外科2016年1月～2025年1月收治并行手术治疗的45例颅骨生长性骨折的患儿临床资料,对其临床特点进行总结分析。结果 45例颅骨生长性骨折患儿,既往均合并颅脑外伤病史,发病年龄6月～8岁2月。临床表现主要是头部局部包块、颅骨缺损、神经功能障碍或癫痫。病变位置主要是额骨、颞骨及顶骨。全部患儿经术前评估后接受外科手术治疗,术中发现硬脑膜撕裂并同时进行水密缝合。术后均恢复良好。结论儿童颅骨生长性骨折的治疗应做到早期干预、积极手术,外科手术治疗效果确切,预后良好。严密修复硬脑膜是手术成功的关键。

Abstract: Objective To analysis the pathogenesis,clinical characteristics,and surgical prognosis of growing skull fracture in children. Methods By collecting and analysising the clinical datas of children with growing skull fracture from 2016.01 to 2025.01 in the Department of Neurosurgery in Xi'an Children's Hospital,numbered 45 cases,summarizes the characteristics. Results The 45 cases of children with growing skull fracture were suffering form Craniocerebral injury,with an onset age of 6 months to 8 years and 2 months.Themain clinical manifestations were local mass,skull defect,neurological dysfunction or epilepsy.The main lesion location in frontal bone,temporal bone and parietal bone.All children received surgical treatment after preoperative evaluation and got excellent prognosisafter surgery. Conclusion Early intervention and positive operation should be taken in the treatment of chillren with growing skull fracture,and the surgical treatment is effective and the prognosis is good.The key procedure in the success of the operation was repairing dural closely.
