蓝牙可感知3.0T核磁兼容脑深部电刺激术治疗帕金森病的临床应用(附3例病例分析)

Clinical Application of Bluetooth-Sensible 3.0T MRI-Compatible Deep Brain Stimulation in the Treatment of Parkinson's Disease (with analysis of Three cases)

摘要: 目的 探讨蓝牙可感知3.0T核磁兼容脑深部刺激系统在帕金森病丘脑底核刺激术中的临床应用及治疗机制。方法 收集2023年7月至2024年1月在中国科学技术大学附属第一医院(安徽省立医院)神经外科行蓝牙可感知3.0T核磁兼容脑深部刺激系统治疗的帕金森病患者3例的临床资料,术后随访11~18月,分析患者临床疗效、程控情况、脑影像和功能的改变情况,并分析术后药物关期各触点电生理信号在程控中的指导作用。结果 3例患者均手术成功,术后影像显示电极位置准确,随访时运动症状改善52.94%、62.74%、80.85%,刺激触点与核团最强β振荡触点一致性强,脑功能分析显示皮质-基底节-脑干-小脑纤维束连接增强、左侧辅助运动区与左右侧感觉运动区、额顶叶、脑干及部分小脑区域存在广泛功能连接增强。结论 蓝牙可感知3.0T MRI兼容脑深部刺激设备可在术后采集各触点电生理信号,既可辅助帕金森病患者术后程控,也为探索脑深部电刺激术治疗帕金森病的脑功能学机制提供重要价值。

Abstract: Objective To investigate the clinical application and therapeutic mechanisms of a Bluetooth-sensible 3.0T MRI-compatible deep brain stimulation (DBS) system in subthalamic nucleus stimulation for Parkinson's disease (PD). Methods Clinical data from three PD patients treated with the Bluetooth-sensible 3.0T MRI-compatible DBS system at the Department of Neurosurgery,First Affiliated Hospital of the University of Science and Technology of China (Anhui Provincial Hospital) from 2023 July 4 to 2024 January 8 were collected.Follow-up continued until December 2024,assessing clinical efficacy,programming adjustments,changes in brain imaging and function,and the guiding role of electrophysiological signals from various contacts during the drug-off phase postoperatively. Results All three patients underwent successful surgery,with postoperative imaging confirming accurate electrode placement.At follow-up,motor symptoms improved by 52.94%,62.74%,and 80.85%.There was a strong consistency between stimulation contacts and the points of strongest β-oscillation in the nucleus.Brain function analysis revealed enhanced connectivity between the cortex-basal ganglia-brainstem-cerebellar fiber tracts and extensive functional connectivity enhancements between the left supplementary motor area,bilateral sensorimotor areas,frontoparietal lobes,brainstem,and certain cerebellar regions. Conclusion The Bluetooth-sensible 3.0T MRI-compatible DBS device allows for postoperative collection of electrophysiological signals from various contacts,assisting in postoperative programming for PD patients and providing significant value in exploring the neurofunctional mechanisms of DBS treatment for Parkinson's disease.
