异柠檬酸脱氢酶1R132H与同源框C10在脑胶质瘤组织的表达及其临床意义

Expression and clinical significance of isocitrate dehydrogenase 1R132H and homeobox C10 in glioma tissue

摘要: 目的 探讨分析异柠檬酸脱氢酶1R132H与同源框C10在脑胶质瘤组织的表达及其临床意义。方法 选取2018年10月至2020年10月保定市第七医院收治的手术切除的128例脑胶质瘤患者,取其胶质瘤组织标本128份。采用免疫组织化学法检测胶质瘤组织中和正常脑组织中异柠檬酸脱氢酶1(isocitrate dehydrogenase,IDH1)R132H与同源框C10(Homeobox C10,HOXC10)表达情况。分析IDH1R132H、HOXC10表达与脑胶质瘤临床病理特征及预后的关系。结果 IDH1R132H在低级别脑胶质瘤中阳性率为63.46%,在高级别脑胶质瘤中阳性率为14.47%,高级别脑胶质瘤中阳性率低于低级别脑胶质瘤;HOXC10在低级别脑胶质瘤中阳性率为46.15%,在高级别脑胶质瘤中阳性率为77.63%,高级别脑胶质瘤中阳性率高于低级别脑胶质瘤,IDH1R132H、HOXC10表达与脑胶质瘤病理级别有关(P<0.05)。Spearman等级相关分析结果显示,脑胶质瘤中IDH1R132H表达与HOXC10表达呈负相关关系(r=-0.457,P<0.05)。年龄、病理级别、术前卡氏功能评分、术后放化疗、IDH1R132H表达和HOXC10表达与脑胶质瘤患者预后有关(P<0.05);年龄≤45、病理级别低级别、术前卡氏功能评分高、术后放化疗、IDH1R132H阳性患者预后较好(P<0.05),HOXC10阳性患者预后较差(P<0.05)。将有统计学意义的因素纳入多因素Cox回归分析,结果显示,病理级别(HR=5.249,95%CI:2.041~13.500)、术后放化疗(HR=4.899,95%CI:2.482~9.671)、HOXC10阳性(HR=2.593,95%CI:1.592~4.225)是影响脑胶质瘤患者预后的独立危险因素(P<0.05)。结论 脑胶质瘤组织中IDH1R132H呈较低表达,HOXC10呈较高表达,二者在脑胶质瘤中表达呈负相关,其表达与患者预后密切相关,可作为预后评估的重要参考指标。

Abstract: Objective To investigate the expression and clinical significance of isocitrate dehydrogenase 1R132H and homeobox C10 in glioma tissue.Methods 128 patients with gliomas who underwent surgical resection in Baoding Seventh Hospital from October 2018 to October 2020 were selected,and 128 glioma tissue specimens were collected.Immunohistochemistry was used to detect the expression of isocitrate dehydrogenase(IDH1) R132H and Homeobox C10(HOXC10) in glioma tissue and normal brain tissue.Relationship between the expression of IDH1R132H and HOXC10 and the clinicopathological characteristics and prognosis of glioma was analyzed.Results The positive rate of IDH1R132H was 63.46% in low-grade gliomas and 14.47% in high-grade gliomas,and the positive rate in high-grade gliomas was lower than that in low-grade gliomas;The positive rate was 46.15% in high-grade gliomas,and 77.63% in high-grade gliomas.The positive rate in high-grade gliomas was higher than that in low-grade gliomas.The pathological grade of glioma was related(P<0.05).The Results of Spearman rank correlation analysis showed that the expression of IDH1R132H in glioma was negatively correlated with the expression of HOXC10(r=-0.457,P<0.05).Age,pathological grade,preoperative Karnofsky function score,postoperative chemoradiotherapy,IDH1R132H expression and HOXC10 expression were related to the prognosis of patients with glioma(P<0.05);age≤45,low-grade pathological grade,preoperative Karnofsky function Patients with high scores,postoperative radiotherapy and chemotherapy,and IDH1R132H-positive patients had better prognosis(P<0.05),while HOXC10-positive patients had poor prognosis(P<0.05).Statistical factors were included in multivariate Cox regression analysis.The Results showed that pathological grade(HR=5.249,95%CI:2.041~13.500),postoperative chemoradiotherapy(HR=4.899,95%CI:2.482~9.671),HOXC10 positive(HR=2.593,95%CI:1.592~4.225) was an independent risk factor affecting the prognosis of glioma patients(P<0.05).Conclusion The expression of IDH1R132H in glioma tissue is lower,and the expression of HOXC10 is higher.The expression of the two is negatively correlated in glioma,and its expression is closely related to the prognosis of patients,which can be used as an important reference index for prognosis evaluation.
